Squamous cell carcinoma surgery in Melbourne

Dr Brian Yue treats cutaneous squamous cell carcinoma, or SCC, as part of his skin cancer and reconstructive surgery practice in Melbourne. SCC can grow locally and, in some cases, spread to lymph nodes or other sites. Its behaviour depends on factors including the site, size, depth, pathology findings, previous treatment and immune status.

Understanding the risk of an SCC

I assess the lesion and pathology together. A small surface lesion may still require careful planning if it lies near an important structure or has concerning microscopic features. Where appropriate, examination of regional lymph nodes and further investigations contribute to the treatment plan.

Excision and reconstruction

Surgical treatment removes the cancer with a margin of surrounding tissue, followed by pathological assessment. The size and depth of the excision are determined by the tumour and clinical context. Repair may involve direct closure, a skin graft or a flap.

Preserving function matters particularly on the face, scalp, ear and hand. I plan the reconstruction around the tissue that must be removed, rather than narrowing an excision simply to make closure easier. If pathology shows incomplete clearance or other concerning features, further surgery, multidisciplinary review or additional treatment may be needed.

Recovery and surveillance

The risks include bleeding, infection, delayed healing, altered sensation, visible scars, graft or flap problems, incomplete clearance and recurrence. The extent of surgery determines the dressing requirements and restrictions on activity.

Follow-up is tailored to tumour risk and includes reviewing the pathology result. Some patients need coordinated care with other cancer specialists. Seek assessment of a growing, bleeding or non-healing lesion rather than waiting for a routine skin check. Read about skin graft and flap reconstruction for information about wound repair.
