Melanoma surgery in Melbourne
Dr Brian Yue provides melanoma excision and reconstruction within his Melbourne plastic surgery practice, with referral to a specialist melanoma team when staging or additional treatment is needed. Melanoma care begins with an accurate diagnosis and a plan based on the pathology, rather than the appearance of the mark alone.
Treatment guided by the pathology
The report distinguishes melanoma confined to the outer skin layer from invasive melanoma. Features such as Breslow thickness and ulceration help determine the next steps. Bring the biopsy report and information about the biopsy site to your appointment.
Wide local excision
After a diagnostic biopsy, further removal of skin around the site is commonly recommended even if no visible melanoma remains. This is called wide local excision. The required margin depends on the diagnosis and tumour features. The tissue is examined by a pathologist, and further treatment may be required if clearance is inadequate.
Lymph node assessment and specialist coordination
Some patients need discussion of sentinel lymph node biopsy to assess possible microscopic spread. This is not necessary for every melanoma. When relevant, the discussion should happen before definitive excision and major reconstruction so the sequence of treatment can be coordinated. Sentinel node assessment, imaging and systemic treatments are arranged through the appropriate specialist team.
Repair and recovery
The wound may close directly or require a skin graft or flap . Reconstruction follows the cancer treatment plan, with attention to function, appearance and subsequent surveillance. Risks include bleeding, infection, wound or graft failure, scarring and the possibility of further surgery. A local operation does not remove the need for appropriate melanoma follow-up.
Follow-up depends on the stage and individual risk. It may involve skin examination, lymph node surveillance and other investigations or treatments. A new or changing lesion should be assessed promptly; do not rely on a website photograph to decide whether a mole is melanoma.
